Filing a Non-Personalized Goods and Service Tax (GST) / Harmonized Sales Tax (HST) Form

​​​​The GST / HST charged and paid on expenses is traditionally reported by a registrant using the personalized return, GST34, provided by the Canada Revenue Agency (CRA). The return includes pre-printed information of your business, including an access code to file the return.

If a personalized return form is not received from the CRA or is misplaced, a standard return Form GST62, GST / HST (non-personalized) may be used instead. The form is to be completed by the registrant and sent to the CRA.

The form GST62 is not available from the CRA website for download. The non-personalized return must be ordered from the CRA’s’ website or by calling the CRA’s forms line. Ordering the form may cause delays in filing the return if there are delays in receiving the form from the CRA.
